Disclosure

CleoHR, Inc. ("Passport") may obtain information about you from a consumer reporting agency for employment purposes. This means Passport may obtain one or more reports regarding your background, including but not limited to: identity verification, criminal history, employment and education history verification, and other information bearing on your character, general reputation, and mode of living, in connection with your application for a Passport and any jobs you apply to.

Your rights

• You have the right to request a copy of any background report obtained about you.
• You have the right to dispute the accuracy or completeness of any information in your report directly with the consumer reporting agency that furnished it.
• A summary of your rights under the FCRA will be provided to you separately, as required by law.

Authorization

By checking the box below and continuing, I authorize CleoHR, Inc. and its service providers to obtain the consumer reports described above for employment purposes. I understand this authorization is voluntary, and that a copy of this authorization is as valid as the original.
